Care guide / Question worksheet
Look up the names. Keep the answers.
Match the names you are given to public board records. Then compare what each option has actually explained.
If this is where you are
The websites look polished. You have a few answers from different care options, but their badges and headings make them hard to compare.
Check names against public records
Public doctor licence records come from state medical boards. The boards also publish disciplinary information: official penalties or restrictions on a doctor. What they show differs by state.
Medical-board federation / What public records can showThe medical-board federation has a directory that links to each state’s medical and osteopathic boards.
Find the board for your stateAn osteopathic doctor is another type of doctor. Some states keep those licence records with a separate board. Ask which board to search, then match the full name and licence details.
A record with no public discipline shows no published penalty or restriction on the doctor. It cannot show the quality of that doctor’s care.
Medical-board federation / Read the record within its limitsAsk who makes and sends the medicine
The US agency that reviews medicines is the Food and Drug Administration, or FDA.
Write down the pharmacy name from the answer. Keep the pharmacy’s name apart from the service name used to arrange the visit.
The FDA provides links so readers can search state pharmacy licence records for the named pharmacy.
FDA / Open your state’s pharmacy searchAs of September 26, 2026, FDA registration meant putting a business on the agency’s records. A drug listing put a medicine on those records. Neither was proof of FDA approval for the business or the medicine.
FDA / The reach and limits of approvalAsk for a direct answer about the medicine, rather than accepting a badge as the whole answer. If a name or record does not match, write down what needs to be cleared up.

Use your own short labels for the options. This sheet does not rank them or decide who can treat you.
